So much of my practice comes down to a single idea: giving the body what it needs to heal itself. Shockwave therapy fits that philosophy beautifully. It is a non-invasive treatment that uses gentle acoustic pressure waves to reawaken the body’s own repair response in tendons, ligaments, and other soft tissues that have become stuck in a chronic, painful cycle. There are no needles, no incisions, and no significant downtime—just a targeted nudge that encourages your tissue to begin healing again.

What Is Shockwave Therapy?

Shockwave therapy—also called extracorporeal shockwave therapy, or ESWT—uses short, high-energy sound waves that travel through the skin and into injured tissue. Despite the name, there is no electrical shock involved. It is a mechanical pressure wave, and its purpose is simply to stimulate your body’s natural healing response in an area that has stalled.

How Shockwave Therapy Works

When these acoustic waves pass through injured tissue, they create a controlled, healthy kind of mechanical stress. Research suggests this can:

  • Encourage new blood vessel growth, improving circulation to tissue that has been chronically starved of it
  • Activate the cells responsible for repair, including the fibroblasts that build collagen and the tenocytes that rebuild tendons
  • Support collagen production, helping to rebuild stronger, better-organized tendon and ligament tissue
  • Help break up the calcium deposits found in conditions such as calcific shoulder tendinitis
  • Calm pain by quieting the nerve endings that transmit it
  • Restart a natural inflammatory response, which can “wake up” healing in tissue that has been stuck in a chronic state for months

Unlike a cortisone injection, which mainly suppresses inflammation, shockwave is designed to stimulate genuine repair. Shockwave has the strongest research support for chronic tendon and soft-tissue problems that have not responded to rest or other conservative care. I most often use it for:

What to Expect During Treatment

Shockwave treatment is straightforward and done right here in my office:

  • I apply a gel to the skin over the area we are treating.
  • A handheld applicator delivers several thousand gentle pulses to the tissue.
  • You will feel a tapping sensation that can become briefly sharp over the most injured spots—this usually tells us we have found the right area.
  • Most sessions take only 5 to 15 minutes.

A typical course is three to five treatments, spaced about a week apart, though I will tailor the plan to your specific condition and how you respond.

Focused vs. Radial Shockwave

There are two main types of shockwave, and the right one depends on your injury:

  • Focused shockwave delivers greater energy to deeper tissue and is often the better choice for stubborn tendon injuries and certain bone-related problems.
  • Radial shockwave spreads outward from the applicator and works well for more superficial tendon and muscle conditions.

Matching the type of shockwave—and the energy level—to your specific injury matters far more than simply turning the settings up. My goal is always to deliver enough energy to stimulate healing while keeping treatment comfortable and precisely targeted to the tissue that needs it.

Is Shockwave Therapy Right for You?

Shockwave is safe and well-tolerated for most people, but it is not appropriate for everyone. I generally avoid it over cancerous tumors, active infections, a growing fetus during pregnancy, or open growth plates in children, and I take extra care with anyone who has a significant bleeding disorder or takes certain blood-thinning medications. That is why every treatment plan begins with a thorough evaluation of your history and your specific injury—so we can be confident shockwave is the right fit before we begin.

In my experience, shockwave works best when it is one piece of a larger picture rather than a stand-alone fix. I see the strongest results when I pair it with progressive loading and rehabilitation exercises, hands-on manual therapy, correction of the biomechanical habits that created the problem in the first place, and honest education about activity and recovery. Treating the whole person—not just the sore spot—is how lasting healing happens, and it is the approach I bring to every patient I see.
